Patients can be referred to the Hospice either by their GP, community nurse specialist, district nurse, hospital doctor or consultant.
You can refer patients for management of symptoms, emotional support, counselling, out-patient support and palliative or end of life care.

Who can make a referral?
- Any health and social care professional who knows the patient can make a referral.
- Self referrals or referrals from family members are also welcomed but we will contact your GP to obtain further information.
How to make a referral

- If referring a family member, we will be asking for their consent.
Referrals for all services come through the Hospice Hub. Patients and their families can contact the team to discuss the services available.
Referrers can expect
- The Hub will make telephone contact with the patient or their referrer within 48 hours to assess if suitable for specialist palliative care services and the urgency of the referral.
- We will contact you to talk about the proposed plan for assessment.
Patients and family members are always welcome to arrange a visit to St Michael’s Hospice and talk to a member of the Hospice care team or get advice at any time.
