You can refer patients for management of symptoms, emotional support, counselling, out-patient support and palliative or end of life care.

Who can make a referral?

  • Any health and social care professional who knows the patient can make a referral.
  • Self referrals or referrals from family members are also welcomed but we will contact your GP to obtain further information.

How to make a referral

Referrals for all services come through the Hospice Hub. Patients and their families can contact the team to discuss the services available.

Referrers can expect

  • The Hub will make telephone contact with the patient or their referrer within 48 hours to assess if suitable for specialist palliative care services and the urgency of the referral.
  • We will contact you to talk about the proposed plan for assessment.

Patients and family members are always welcome to arrange a visit to St Michael’s Hospice and talk to a member of the Hospice care team or get advice at any time.
