We’re here to provide compassionate care and support. Our services are dedicated to adults facing serious progressive illnesses, aiming to enhance their quality of life from diagnosis through treatment and until their final days. We extend our care to family members and those who are bereaved, offering a supportive network throughout their journey. In many cases, our focus shifts from curative to palliative care, especially for patients with advanced, progressive diseases.
For some individuals with complex needs, we are there to assist even earlier in their disease journey. Your well-being is our priority, and we’re here to lend a helping hand whenever you need it.

Who can make a referral?
Patients can be referred to the Hospice either by their GP, community nurse specialist, district nurse, hospital doctor or consultant.
When is a referral needed?
Referral is suitable in situations where:
– Pain or other symptoms remain challenging to manage despite intervention.
– Meeting the psychological, social, and spiritual needs of the patient, family, or significant others becomes a challenge.
– Additional support is needed for end-of-life care in the final days.
How to make a referral
Referrals for all services come through the Hospice Hub. Patients and their families can contact the team to discuss the services available.

If referring a family member, we will be asking for their consent.
What happens next?
The Hub will make telephone contact with the patient or their referrer within 48 hours to assess if suitable for specialist palliative care services and the urgency of the referral.
We will contact you to talk about the proposed plan for assessment.
Patients and family members are always welcome to arrange a visit to St Michael’s Hospice and talk to a member of the Hospice care team or get advice at any time.
