Specialist support for those living with Dementia

At St Michael’s Hospice, we understand the complexities of dementia and are here to support both patients and their families or carers every step of the way.

Our Services

We offer comprehensive support tailored to the unique needs of those affected by dementia:

  • Carer Support: From the moment of diagnosis through to end-of-life care, our team provides personalised assistance to help navigate the challenges of dementia. Whether it’s through home visits or meetings at St Michael’s Hospice, we’re here for you.
  • Ongoing Support: We’re committed to accompanying you on the entire dementia journey, offering continuous assistance and guidance along the way.
  • Access to Wider Services: In addition to dementia-specific support, we provide access to a range of other services at St Michael’s Hospice, including counseling, complementary therapy, mindfulness sessions, spiritual care, and hospice-at-home care.

How to access support

We accept referrals for our dementia and wellbeing services from anyone registered with a Herefordshire GP, including:

  • Carers of individuals with complex needs related to their caring role.
  • Individuals living with dementia with unmet needs related to their diagnosis.
  • Those seeking advance care planning support and advice.

Referrals can be made at any stage of the dementia journey, from diagnosis onwards and during times of change or deterioration. Additionally, our services are available for individuals diagnosed with dementia before the age of 65 years, registered with a Herefordshire GP.
