Introducing our Admission and Discharge Team

At St Michael’s, we are committed to providing compassionate palliative care to people across Herefordshire and the surrounding areas.

It is a myth that ‘you come to a Hospice to die’ – in fact a high percentage of our patients are discharged either home or to another care setting.

We now have a dedicated Admission and Discharge Team… namely Verity and Heather!

They are able to support patients and their families to navigate options available. These discussions are had with and alongside the wider care team on the Inpatient Unit (IPU), who support patients to make decisions about future care and ensure they are made in a safe, timely, and considered way.
