This is an exciting opportunity to join the friendly and multidisciplinary Hospice at Home team. You will be responsible for assessing, planning and delivering a high quality palliative and end of life care to patients and families within their home or care home. The role provides an opportunity to provide support to those most in need and enables patients to stay in their preferred place of care.

You must have a relevant degree or equivalent qualifications, and have experience and knowledge of palliative care.

If you are adaptable, can juggle differing priorities, have excellent communication skills and the ability to not only work within a team but collaboratively with primary and community care colleagues, then this is the role for you.
