As a Hospice at Home Nurse you will use your nursing skills and knowledge, complemented by the education, support and mentoring we will provide, to ensure all patient’s end-of-life care needs are addressed. You will work with multiple agencies, advising, explaining and acting as a patient advocate in conversations with GPs, paramedics and other healthcare providers. This multi-disciplinary collaboration, alongside excellent communications and teamwork within the charity, means your patients will have continuity of care and be able to focus on quality time with their family.

Visiting patients in their own homes, you will be providing expert palliative care alongside emotional and psychological support for the whole family. You will be building a rapport over weeks, months or even years so that symptoms are controlled, hospital admissions prevented and families able to spend quality time together, at home.

As well as joining an enthusiastic friendly and committed team, our location is hard to beat – nestled in the stunning Herefordshire countryside, in a state-of-the-art facility with free on-site parking, discounted delicious meals snacks and drinks.
