It is a joint clinic run at St Michael’s Hospice with renal consultant Martin Ferring from Hereford and Worcester hospitals. The clinic supports patients from both a renal and palliative point of view. The general age group of those attending is over 75 years old, although this is not the only deciding factor; things such as frailty and co-morbidity are also taken into account.
Referral to the clinic is usually via a GP or the kidney clinic consultant. If patients are too frail to attend the clinic, they are supported by phone and correspondence as well as by the information and support being offered by the Hospice Team to Primary Care.
