Thank you for taking part in the Venus Challenge Midnight Walk 2010. Please use this form if you are wishing to enter as a team or group. These discounted prices are specified per group as £55 (team of 4), £80 (team of 6), £100 (team of 8), or £120 (team of 10).
To enter just place the number of teams you would like in the ‘Quantity’ box, not the number of members in the team. Don’t forget to fill in all of the form, including each required t-shirt size, and have your payment details ready.
Please book online before midday on Friday 17th September to qualify for pre-entry prices; this will allow the Hospice to process your payment and attend to administration tasks before the event. Payment can be made on the night for £17 per entrant.
Please note: There is a discounted individualised rate (£11 per person) for groups containing 11 or more members. For more details please contact the Events Team on 01432 851000 or download a form and post with names, addresses, phone numbers, email addresses and t-shirt sizes for all entrants, along with a cheque covering total payment made out to “St Michael’s Hospice”, to Venus Challenge, St Michael’s Hospice, Bartestree, Hereford, HR1 4HA.
